Required Documents for those applying for new Submitter IDs

The following documents are required documents that must be completed, signed and returned to the Empire office prior to initiation of electronic claims submission or inquiry.

EDI Registration Form

Section I-III

  • Select Add and choose your Line of Business
  • Select 837 Claim, 835 Remittance, and EDI Reports

Section IV-V

  • Leave the Network Connectivity Section Blank
  • In the next section, choose "SFTP"

Section VI-VII

  • Enter your Provider or Business/Practice Name and leave the Submitter ID blank
  • Choose Operating as "Provider"
    • If you are a billing service, choose "Billing Agent"
  • Enter your Billing Address
  • Enter your Phone and Fax numbers
  • Enter the name of the Primary Contact person for your office
  • For the Software Vendor section, please enter the information for AXIOM Systems, Inc. (formerly Ivertex) that's provided on the table above.

Section VIII

  • List the names and ID numbers of the providers you will be billing for

Empire Blue Trading Partner Agreement

  • Check the box to Accept the Terms and Agreements
  • Trading Partner is the name of your Business/Practice/Provider Name
    • (the entity requesting a Submitter ID)
  • Enter a valid email address for EDI notifications
  • Enter the name, title, phone and fax number for the main contact in your office
  • Enter your demographic information
  • Enter AXIOM Systems, Inc. for the question that says: "Known Concentrators"
  • Click "Submit"

Submitting your Forms

It is recommended that you keep a copy of all the forms you will be submitting for your records. Mail the EDI Registration enrollment form reflecting original signatures to:

It is very important that you complete and return the entire enrollment packet as described above. Incomplete forms will not be processed and will be returned to the submitter.

Waiting for a Response

Once the complete provider enrollment packet has been received, the documents will be processed. Processing will take approximately two to four weeks from the date of receipt. (Remember that mailing time can take as much as five days.)

After processing, a confirmation will be faxed to you as notification to begin filling claims electronically. If neither confirmation nor a returned packet is received after four weeks, contact the Technology Support Center toll-free at 1-800-470-9630.

Testing

Once you have received your Submitter ID and Password from Empire, please call the SolAce Support Team and set an appointment for a Mailbox setup and Test Transmission to Empire.

Please have a minimum of 10 test claims ready for testing. Test files should consist of a variety of claims that represent the type of claims you will be submitting once production status is achieved. Test claims will not be processed for payment but will be validated against production files; therefore, they must contain valid patient procedure, diagnosis, and provider information.
